YOU MAY COMPLETE THE INFORMATION FOR YOURSELF AND ANY MINOR DEPENDANTS. IF ADDITIONAL DEPENDENTS ARE 18 OR OVER, HIPAA REQUIRES THEY WILL NEED TO COMPLETE THE MEMBERSHIP THEMSELVES.
